You may download and print out the admission application forms. The admissions packet of forms depends on the grade for which your child is applying, either Pre-K / Kindergarten, or Grades 1-7. These forms are in Adobe Acrobat (Pdf) format and you will need Acrobat Reader in order to download these materials. If you don't have Acrobat Reader, you may click on the icon below to download a free copy.

Step 1 - Initial Application Process

• Admission Application Form (completed by you)
• Admissions Process & Policies (for your information)
• Permission & Release of Student Records Form (completed by you)
• Fee Schedule Form (completed by you)
• Confidential School Report, Grades 1-7 (you complete the top portion, you send it to your child's previous school; that school returns it directly to the Director of Admissions at RVIS)
• Confidential School Report, Pre-K and Kindergarten (same procedure as above)

• Immunization Record Form (completed by you or your doctor)
• Tuition Authorization Form (completed by you or your employer)
• Treatment & Travel Permission Form (completed by you)
  Emergency Contact Form (completed by you)

Include the application fee of BD50 with your competed application packet. This fee includes the admissions test.
